For a diagnosis of POF, typically there must be an absence of menses for at least 4 months, and abnormally high levels of follicle-stimulating hormone (FSH), determined after two blood tests. Average age of occurrence is 27, but POF can strike in younger women, even teenagers. |
Doctors don't fully understand why ovaries stop functioning normally in women with POF, but they recognize two basic problems:
When no responsive follicles (the sacs which hold the eggs) are left in the ovaries. This may be due to chemotherapy, or disorders such as:
- Chromosomes determine a person's sex. Women have two X chromosomes, but for women with fragile X syndrome, one of the chromosomes is either missing or damaged (this is the case for 2-3% of women with POF).
When follicles are still present in the ovaries, but no longer function. Doctors hypothesize that the following issues can be at play:
- Instead of attacking viruses and bacteria, the immune system will inexplicably attack the body itself, including developing follicles, preventing their maturation. Affects about 5% of women with POF.
- Even if functioning follicles are still present in the ovary, for unknown reasons unusually low levels of follicles may inhibit ovulation. Estimates say this may be the case for up to 60% of women with POF.
- 10-20% of women with POF have a family history of the condition.
